Ritual Request	For Initiates, Artisans, Guardians and Ambassadors to request and describe a R/T Ritual or session in which they will participate or an online (cyber) ritual for viewing at Realmstone.
Explanation of Realmstone Surrender Ritual Request	Explains the purpose of the Ritual Request, how to prepare the form and lists any additional forms that may be required.
D/s Special Event Request	For Initiates, Artisans, Guardians and Ambassadors to request and describe a non-ritual R/T event or session in which they will participate or for their viewing at Realmstone.
Explanation of D/s Special Event Request	Explains the purpose of the D/s Special Event Request, how to prepare the form and lists any additional forms that may be required.
Narrative Request	For any Realmian to request and describe an online or snail mail ritual narrative. A narrative is a text and photo story of a ritual.
Phone Training Request	For any Realmian to request a phone training session.
Realmstone Temple Reservation	For Initiates, Artisans, Guardians and Ambassadors to reserve the Realmstone Temple for their own use.
Ritual or Event Attendance Request	For Initiates, Artisans, Guardians and Ambassadors to request attendance (not participation) at a Realmstone ritual or other event.
Request to Visit Realmstone	For Initiates, Artisans, Guardians and Ambassadors to request a visit to the Realmstone Temple.
Participant Release and Statement of Age (for models, photographers and videographers)	Must accompany all photo or video submissions for publication in Realmstone. There must be one for each person portrayed and for each photographer or videographer. Must also be submitted with original art when a model posed for the artwork.
Age Verification (Photo ID)	Must accompany all photo or video submissions for publication in Realmstone.
